HUMAN ERROR IN THE WORKPLACE

First of all, we are all humans, and humans aren’t perfect.  We often hear that the cause of an accident was “human error.”  After investigation, that may be found to be true.  But there is a correlation between the workplace environment and the human that is employed there.  We know that businesses must comply with government regulations and standards of safety and health for their employees. 

It would seem that the starting point of avoiding human error is to establish strong policies and procedures.  Beginning with human resources personnel, who pass on valuable information to new employees, and following through with training, management, good communication, and the design of a workplace that leads to safety – are all  factors that will ensure that  people work successfully.  

I can truthfully admit that as a new employee, with early-on training, I still didn’t understand fully about my new job.  It takes time and practice to really catch on.  Maybe some are faster learners that I was, but I know that when there’s too much information to absorb, it’s easy to make errors.  I did clerical work, and there’s certainly room for error there.  Think about someone who does data entry all day long, and may miss just one digit on the keyboard, causing costly errors.  (Remember the computer glitch that caused the Stock Exchange to seemingly tank not too long ago?)  Between 70% and 90% of workplace errors are attributed to “human error”, but it may be due to a mismatch between the plan of the systems with which they may be required to work and the way they think and work. 

If a company assures their employees that they want an employee to report a mistake they made, in order to ensure safety, without reprimanded, those workers will be more likely to do so.  Many times workers are under peer pressure, or demands of their supervisors to complete a job hurriedly; or they may be placed in a work environment that is not conducive to safety.  The blame does not fall on the employee under these conditions.  For example, if their job requires charting or reading meters, and the lighting isn’t adequate, whose fault is it when the readings  are incorrect?  

Companies must have a strong safety committee that enforces compliance with safety rules.  If employees know that  part of their job evaluation is based on their compliance with safety rules and wearing the Personal Protective Equipment they are required to wear, they may be more vigilant to avoid making mistakes.  Continued safety training and repeated training is important to the success of any employer.  Another key to good safety practice is to have some type of reward for employees who recognize and report a potential hazard.  If employers could rotate the repetitive and boring types of responsibilities among several persons, by giving them different tasks, a safer workplace could be established.  Posters always play a valuable role in reminding workers to stay alert. 

As stated earlier, there will always be “human errors.”  Some of those errors have proved to be very devastating to  individuals and families, such as airline crashes or medical errors.  These are usually the ones that are reported to the public.  But small mistakes or big ones will continue being made.  Every single person needs to be more aware of the consequences of their mistakes, and do the very best they can do.

OCTOBER 3 – 9 IS FIRE PREVENTION WEEK

The National Fire Protection Association has been the official sponsor of Fire Prevention Week for eighty-eight years.  This year’s focus is promoting and educating the public on the use of smoke alarms.  The theme for 2010 is “Smoke Alarms: A Sound You Can Live With.” 

Many homes have smoke alarms, but how many of us take the recommended once a month time to check them out? According to the NFPA, there should be a smoke alarm in every bedroom, outside all sleeping areas, on every level, even the basement.  Most homes do not contain that many smoke alarms.  Families should all be aware of escape plans in case of fire. Two/thirds of home fire deaths were a result of fires in the home that either had no smoke detectors at all, or they did not function properly.  NFPA data shows that smoke alarms that work correctly can cut the chance of dying in a home fire by 50%.  You can find more information at the website: http://www.firepreventionweek.org./ If you have smoke alarms and they begin to “chirp,” they are telling you that they need a new battery! 

There can be multiple causes of fires, either at home, work, on the highways, and other locations where accidents occur.  We depend on our firefighters in times of emergency.  They are trained not only to extinguish and contain fires, but to rescue civilians and furnish medical assistance.  Some are paid and others are volunteers.  These public servants are available 24 hours a day, 7 days per week.  

We owe it to these men and women to be cautious when camping outdoors.  Wildfires can happen quickly, and cause devastation to homes, humans, wildlife, and miles of terrain.  Firefighters put their lives on the line many times because of someone’s carelessness.  In addition to the responsibilities firemen and women have, they must wear heavy duty PPE, as well.  Like our military, fire personnel have to wear heavy equipment, such as turnout jacket and pants, safety boots, safety glasses, SCBA (self-contained breathing apparatus), helmet with face mask/visor, special gloves, a PASS device (personal alert safety system), and reflective vests over their turnout jackets when helping with accident control.

They also conduct inspections of buildings for violations of fire codes.  After a fire, it’s up to fire personnel to conduct investigations to determine the cause.   If you are concerned about the safety of your home, or plan to purchase a home, your fire department will send someone to check it out for you.  Fire departments are helpful to citizens in many ways, such as conducting classes in fire prevention. 

The next time you see a fireman, thank him/her for what they do.  It’s a job that requires a special breed of people, ones who are willing to serve whenever called on.  And remember, think safety always and help prevent fires.

HANDLING WORKPLACE VIOLENCE

Although we have addressed this subject in an earlier article, Violence in the Workplace, it is such an important issue, that we want to talk about it at a different angle.  Both articles contain information that may be helpful, should you find yourself in such a situation.  According to OSHA, violence in the workplace is the fourth leading cause of fatal occupational injuries in the United States, posing a very serious safety and health issue. 

It is critical that employers have a comprehensive workplace violence policy in place.  Hazard assessments must be done, as well as identifying risk factors.  The training program should include every employee, being sure they understand how important it is to report any incidents that could possibly cause a reaction resulting in harm to other employees.  Disruptive behavior should not be tolerated.  Yelling, profanity, waving arms and fists, verbally abusing and refusing reasonable requests from the supervisor to cease such acts are signs of being disruptive.  Threatening physical actions, short of actual contact, such as moving closer to the person, oral or written threats or comments like “you’ll be sorry”, should not be tolerated.  Violent behavior is when a person physically assaults another person, with or without weapons.  People prone to violence throw things, pound on desks, doors, destroy property, and may threaten to shoot or harm others.  (Sure hope you don’t know anyone who acts this way!) 

Managers and staff must set and enforce standards of conduct and be there to help with calming situations down if at all possible.  Suggestions are to talk to the person, but not isolate oneself.  Using a calm approach may be helpful.  Never touch the person, as that could trigger a violent reaction.  Have a signal set up to let other coworkers to know that you need help (so they can call 9-1-1.)  Waving or getting the offenders’ attention may set him off.  If you feel that your intervention is not helping, leave the room, and call the police, if it hasn’t already been done.  Get yourself and others to safety.  Leave an open line for the police if possible, so they are better informed of the confrontation. 

Once a situation is defused, hopefully without violence, those who served as witnesses, potential victims, and the families and friends of all involved will have a certain amount of trauma to deal with.  The company should have counseling and interventions for all to be able to participate. 

Hopefully, you will never have to deal with anything like this where you work.  If you witness anything that you suspect could escalate to any type of disruptive behavior, tell your supervisor.  Many times there are warning signs that we see as not serious; however, the well-being of all your coworkers may be at risk.  A better way of resolving conflict among workers could be served by managers and staff who are willing to have open communication with all employees at all times.

THE IMPORTANCE OF BEING PHYSICALLY FIT

Because the month of May is National Physical Fitness and Sports Month, we want to talk about how important it is to take care of our bodies by being active.  President Dwight Eisenhower started the Presidents Council on Youth Fitness back in 1956, emphasizing the importance of ensuring that our youngsters were as physically fit as their European counterparts.  Through the years, different Presidents have given the project various titles, always emphasizing fitness of young people, followed later by promoting physical fitness of all ages.

According to findings by the following: The U.S. Department of Health and Human Services, Centers for Disease Control and Prevention, National Center for Health Statistics, Office of the Surgeon General, Call to Action to Prevent and Decrease Overweight and Obesity, and the Office of Disease Prevention and Health Promotion, we want to highlight some valuable information.

  • The ideal requirement for adults (18 or older) is 30 minutes of physical activity for 5 or more days per week, and children and teens should get 60 minutes of activity every day for their health.  Thirty to sixty minutes of activity may be broken into smaller segments of 10 or 15 minutes throughout the day to count toward your total exercise time.
  • Physically inactive people are twice as likely to develop coronary heart disease, the leading cause of death among U.S. men and women.
  • Inactivity and poor diet can lead to overweight/obesity.  This increases the risk for heart disease, high blood pressure, stroke, gallbladder, respiratory, diabetes, and other serious health problems.  Nearly 60 million Americans are obese.  Sixteen per cent of children and teens aged 6 to 19 are now 3 times more overweight in 2002, than in 1980.  The number of overweight children ages 2 to 5 has doubled since 1980.
  • Not all high school students participate in regular school physical education.  It is important that schools emphasize the need for physical education.
  • A study done by the National Association of Sports and Physical Education (NASPE,) shows that infants, toddlers, and pre-schoolers should engage in at least 60 minutes of physical activity daily, and should not be sedentary for more than 60 minutes at a time except when sleeping.
  • One-fourth of U.S. children spend 4 hours or more watching television daily.

In the old days, we would play outside with neighborhood kids until our parents called us in for supper (dinner).  We rode our bikes, roller-skated, played hopscotch, and did all kinds of activities, simply having fun, not knowing it would possibly have health benefits in later years.

Not all kids are into sports, but parents can do things with them, like taking them bowling, playing miniature golf, or doing other fun things that keep them moving.  It’s good for mom and dad, too!  By showing them there’s more to life than playing video games for hours, texting on a cell phone all day, or sitting in front of the television or computer, they will develop healthy habits for the rest of their lives.

It’s never too late for us grownups to locate a fitness center.  Just taking thirty or forty minutes out of your day to work out, or take a daily walk (cost is free), will be worth the effort.  Many companies and schools provide places for employees and students to exercise.  You don’t have to be a professional athlete, just get out those jogging shoes or barbells and see if you don’t feel better!  Last, but not least, choose healthy foods.
